Please use the St Vincent’s Private Hospital & Day Surgery Unit Booking Form to book and transfer, medical or surgical cases.

Booking forms should be sent to the Booking Office as soon as possible. Please do not fax Booking Forms, as in this process the consent will be lost.  For this reason, faxed Booking Forms will not be processed.

SVPH Hospital Bookings

Booking Form ONLY required to be sent to the Booking Office. (The hospital mails out the Medical History & Pre-admission Information to the patient.)

ADMISSIONS TO ST VINCENT'S PRIVATE HOSPITAL WITHIN 7 DAYS
Bookings will be accepted by telephoning the Booking Office Staff on (02) 8382 7463 or
(02) 8382 7464.  (Please have all the information ready, and the original Booking Form must be sent to the Booking Office at least 48 hours prior to the patient's admission.)

Day Surgery Unit Bookings

Booking Form only required to be sent to the Booking Office ASAP. 

ADMISSIONS TO DAY SURGERY UNIT WITHIN 7 DAYS 
Bookings will be accepted by telephoning the Day Surgery staff directly on (02) 8382 6333.
(Please have all the information ready, and the original Booking Form must be sent to the Booking Office at least 48 hours prior to the patient's admission.)

Day Surgery Unit - all other Bookings

Doctors NOT onsite at St Vincent’s Clinic – Day Surgery Booking ONLY
Please mail out or give the patient the Preadmission Information, Medical History forms & reply-paid envelope to complete and mail back by return post. If they can complete the paperwork in the surgery it can be sent in with the Booking Form.
